Tuttnauer Expands Medical Autoclave Market To China

In April, Tuttnauer attended the CMEF show in Shenzhen, China. This medical device trade show offers a glimpse at the wide array of local and international manufacturers who market their wares in that country. A safety lock prevents the unit from being opened when the chamber is pressurized. No cycle can begin until the machine is fully closed and sealed; steam will not be released into the chamber unless the door is shut. In addition, an obstruction detection system prevents the doors from closing if an object (or an operator’s hand) is in the path.

Rodwell Promotes Vacuum Autoclaves At ACHEMA

Rodwell Scientific Instruments made the trip from Great Britain to Germany for the 2009 ACHEMA show in May. The company was one of over a dozen presenters specializing in medical and laboratory sterilizers. What sets this manufacturer apart is the Rodwell “Pneu-valve” system offered on all their vacuum autoclaves.

Crystal

Ambassador

This is one of the manufacturer’s mid-sized sterilizers at a maximum of 158 liters. The Ambassador features Rodwell’s MP 25 controller. This computer system is fully interchangeable and can work with any autoclave the company offers. It can be programmed with 25 different cycles including those that require delayed start and free steaming adjustments.

Belimed Revamps Central Sterile Supply Department

Earlier this year, Belimed Group established a 10 year finance contract with UKE (Hamburg-Eppenddorf University Hospital). This project involves the installation of all the new sterilization equipment in the hospital’s Central Sterile Supply Department (CSSD).  It is a lease agreement with an option to purchase after 10 years.  Belimed not only supplies the financing but also includes sterilizers, washers, disinfectors, packing tables, furniture, and all the maintenance and service for the equipment. 

This Belimed deal is a small part of a 188 million Euro hospital building project. Steris Introduces New Disinfecting Agent

On June 19, 2009, Steris Corporation announced another new product: Resert  XL HLD High Level Disinfectant.  This broad based microbicide was developed with the help of Virox Technologies using their proprietary AHP ™ (Accelerated Hydrogen Peroxide) formulation.  With one rinse, Resert accomplishes its sanitizing task without leaving any cytotoxic residue. It also doesn’t prompt the formation of biofilm on medical instruments.

This non-aldehyde disinfecting agent works because hydrogen peroxide is the active ingredient and oxidizer.  It’s a reliable and versatile alternative to glutaraldehyde.  This type of product is much safer for patients and staff than the prevalent alternatives.  Virtually no inhalation risk and a milder odor means there are far fewer sensitivity issues. Fumes are almost completely eliminated.  Because of this, anaphylaxis and other severe reactions are not a serious concern when using instruments sanitized with Resert.

The effectiveness of the chemical solution remains strong for up to 21 days from the first use. It can be reused several times over that period which reduces the cost-per-use considerably. It ends up having about the same value per unit as other, equivalent processes.   Cycle time is reduced from a 20 minute soak to only 8 minutes at 68° F. No additional heat is needed in automated processors.  This reduces energy costs. Instrument turnaround is also dramatically improved.  Surgeons are more likely to receive their aseptic instruments on time and when they need them.  This, in turn, can increase morale for the entire OR staff.

Resert is non-toxic and environmentally friendly.  Mixing and measuring are not required because of the prepared nature of the product.  It is easy to use. There are no venting or special disposal procedures. The cleaning agent can safely be rinsed down the drain.

During last month’s ACHEMA 2009 in Frankfurt Germany, Getinge Infection Control introduced three new sterilization products. This Swedish manufacturer was one of over a dozen sterilizer companies that took advantage of this world class event to show off their finest products. Getinge’s offerings included the brand new GEV series, the STERBOX E-Beam system, and the ISOTEST.

GEV

It is a face to face work station where one or two operators use sleeves and ready-to-fit sterile gloves to perform the tests.  A Millipore Equinox Steritest pump and DPTE® door for wastes is provided at each work station.  The second station can be configured for direct inoculation testing.

Waste is collected with a chemical protection system for both the unit’s environment and the microbial sample and placed into a special DPTE®-Disposbag.  DPTE® sealed sleeves protect the work space and the Steritest canisters from contamination during the transfer to the incubator.

Aseptic Processing Guidelines News - 2009

In February, AAMI released their updated aseptic processing guidelines. They have adopted the ISO/TC standards and are issuing this new series as a collection at a reduced cost. These ANSI/AAMI 13408 books will also be available for individual purchase. The first section of this new publication deals with the terminal sterilization of medical devices and other items. This final autoclave or low temp sterilizer step is performed on healthcare products in sealed containers prior to issuance by the manufacturer.

However, there are circumstances in which it is not possible to use this method due to packaging or other constraints. In such cases, aseptic processing can be considered as an alternate technique. AAMI’s documentation speaks to the precautions healthcare device manufacturers must take during this type of assembly. Preventing contamination of previously sterilized items as they go through the line can be difficult. Each component must be carefully handled to ensure that the final product remains aseptic. Section 13408-1:2008 provides valuable information on how to develop a comprehensive processing program.

Other sections of the 13408 publication cover clean-in-place and sterilization in place technologies. Lyophilization (freeze drying), filtration and isolator systems are also dealt with in depth. AAMI is anticipating that two new standards will be released by the ISO shortly. One of these new sections will be about the sterile production of items such as pacemakers and heart valves. The other will cover cell-based (isolated) manufacturing - a popular trend in aseptic processing. Approval of these new standards is expected as early as 2011.

2009 Training Opportunities

PDA Training and Research Institute (TRI) has just one aseptic processing course left in 2009 that has not sold out (session #4 in August-September). This class is broken up into 2 five-day sections to allow students to fully explore the presented topics. These intensive training sessions are scheduled a month apart to minimize organizational disruption for companies that choose to send their employees to TRI for training. This course is designed for technicians and supervisors as well as upper management in QC, Engineering, and Manufacturing departments.

The educational method consists of a combination of lectures and over 47 hours of team projects and hands-on training in a laboratory setting. Participants are expected to come away with practical knowledge that can be applied immediately to improve processes within their own organizations.

This course teaches such basics as facility design, airflow, sanitization, and other factors that affect aseptic processing. Knowledge of isolator technology, gowning, environmental monitoring, and other important functions will be tested as students perform media fills in TRI’s high-tech labs. Participants will learn how to sterilize and prepare components to reduce the risk of product contamination. They will also be taught proper documentation and final testing procedures to verify sterility.

Although aseptic processing can be a powerful tool, terminal sterilization is still the best option for achieving maximum microbe reduction. Alfa Wassermann Separation Technologies initially developed a sanitization and disinfection kit to be used with their pharmaceutical grade ultracentrifuge.  In May 2009, the company reported an important advance. They have determined that this technology can actually be used for the sterilization of a wide variety of other heat sensitive instruments.  Filling machines, filtration systems, laboratory centrifuges, incubators, and laboratory worktops and walls can all be properly cleaned using this system.

The AlfaSIP-50D sanitizes in just 5 to 10 minutes in a centrifuge without any heating or cooling cycles.  Smaller amounts of this solution are needed than comparable disinfecting agents for accomplishing the same job.  This agent comes in packets of 10 kits.  One kit is usually enough to sanitize 8 to 10 loads.  Once the solution is created it remains actively potent for up to 15 days.  This means any extra solution of chlorine dioxide can be temporarily stored for additional applications later.  When the product is used as recommended it can safely treat instruments made of stainless steel, elastomers and many plastics.

A membrane based micro-reactor envelope permits only pure chlorine dioxide gas to diffuse into the water.  This means that the equipment is exposed to 99% pure solution of the active ingredient during the disinfection cycle.  Alfa Wasserman’s patented process virtually guarantees that free chlorine is not produced.  This makes the system both safe and effective.

The flushing during the sanitization procedure ensures that no residue is left behind.  Although the chlorine dioxide is not corrosive to most materials at the doses used, the potent oxidizing agent quickly wipes out any pathogen it comes in contact with.  In fact, ClO2 is more effective than chlorine at disinfecting water and surfaces.  Another advantage is that it is much safer to use than chlorine gas.

Warnings About Chlorine Dioxide

At high enough concentrations in solution ClO2 becomes unstable and will break down into chlorine and O2 very easily.  With 15% or greater solutions at STP, it can be explosive.  Therefore it should never be transported.  It should only be used at the location where it is produced or generated in situ.  Only small amounts can be viably stored on location and only for short periods of time. 

In high enough concentrations, this chemical is harmful to all forms of life.  It is used as an anti-microbial and disinfectant for this reason and the fact that it naturally breaks down.  Adverse effects on humans are still under investigation. Very low doses are generally considered to carry minimal risks.  The benefits of using it often far outweigh any ill effects.  However, direct exposure should be limited.

Short-term exposure symptoms can include watery or irritated eyes and seeing halos around lights. It can also cause irritation of the nose and throat, coughing, and chess pain. The lungs can be irritated resulting in shortness of breath.  Repeated or long-term exposure can cause fluid to build up in the lungs.  High enough exposure can cause pulmonary edema that may not show up for 24 hours after contact.

Astell Introduces Effluent Sterilization System

In May, Astell announced the addition of an effluent sterilization system to its product line. These self contained plants are designed to treat contaminated liquid waste. They are sufficiently effective for use at category 3 and 4 labs. Waste water from designated lab areas in such facilities must be sterilized before discharge into the sewer to comply with environmental regulations. The Astell treatment equipment can be structured to handle capacities of up to 5,000 liters per day. The containment tank receives incoming effluent from sinks and bathrooms. It can also be used to handle exhaust and water discharged from instrument sterilizers and autoclaves.

Chinese Manufacturer Announces Ozone Bed Sterilizer

The ozone bed sterilizer KZ-X-DL1 made in Guang Dong, China has just been released into various markets around the globe over the past few months.  On June 3, 2009, Guangzhou Kangzhen Medical Equipment Co. Ltd announced the increased availability of this portable machine. The manufacturer also took this opportunity to point out some of the equipment’s benefits.

Although the bed and mattress must be encased in a specially sealed plastic bag for the system to work properly, sheets and other items remain in place in the room where they are used by the patient.  Once hooked up, the equipment runs through an automatic cycle that creates its own ozone for sterilization.  Through a set of vacuum and pump phases, deeply embedded pathogens are directly exposed to the ozone and killed.
